Terms & Conditions

At Myrtle Dental Clinic, we are committed to providing safe, ethical, and high-quality dental care. By booking an appointment or receiving treatment at our clinic, you agree to the following terms:

Appointments

  • Appointment times are reserved specifically for you. 
  • We require accurate personal and medical information to ensure safe treatment. 
  • We may contact you by SMS, email, or phone for confirmations or recalls. 

Treatment Plans & Fees

  • Treatment plans and quotes are estimates and may change based on clinical findings. 
  • Any changes will be discussed with you before proceeding. 
  • Fees must be paid on the day of service unless a prior arrangement is approved. 

Health Information

  • You agree to provide accurate medical history and update us if anything changes. 
  • This includes medications, allergies, and health conditions that may affect treatment.

Refund & Cancellation Policy

Cancellations & Missed Appointments

To provide fair access for all patients:

  • We require 24 hours’ notice for cancellations or rescheduling. 
  • Appointments cancelled late or missed may incur a late cancellation fee. 
  • Fees are at the clinic’s discretion based on the appointment length reserved. 

Deposits

For longer appointments (e.g., crowns, root canals, cosmetic dentistry), we may require a deposit to secure your booking.

Refunds

  • Refunds are not provided for services already rendered. 
  • However, if there is a genuine concern about treatment outcomes, we encourage you to contact us so we can provide a resolution. 
  • Refunds for prepaid treatment sessions (e.g., whitening packages) are considered on a case-by-case basis.

Patient Rights & Responsibilities

 Your Rights All patients at Myrtle Dental Clinic have the right to:

  • Receive respectful, compassionate, and culturally sensitive care 
  • Access safe, evidence-based dental treatment 
  • Be informed about treatment options, risks, and costs 
  • Request a second opinion 
  • Access your clinical records upon request 
  • Decline treatment (with an understanding of possible risks) 
  • Have your health information kept confidential

Your Responsibilities To ensure smooth and safe dental care, we ask patients to:

  • Provide accurate health and medical information 
  • Inform us of any changes to your health 
  • Attend appointments on time 
  • Respect cancellation policies 
  • Follow treatment recommendations 
  • Respect staff and uphold a safe environment

Consent to Treatment

By attending the clinic, you acknowledge that:

  • You understand the proposed treatment, risks, alternatives, and costs 
  • You have had the opportunity to ask questions 
  • You can withdraw consent at any time 
  • Some procedures may require a signed consent form (e.g., surgery, extractions) 

Consent may be given verbally, in writing, or implied (e.g., opening your mouth for examination).

Financial & Payment Policy

Payment Requirements

  • Payment is required on the day of service. 
  • We accept EFTPOS, credit cards, HICAPS, private health insurance cards, and Afterpay .

Private Health Insurance

  • Rebates depend on your insurer and coverage. 
  • Patients are responsible for any gap payments.

Government Schemes

We accept:

  • Child Dental Benefits Schedule (CDBS) 
  • DVA 
  • TAC / Workcover (when approved)

Overdue Accounts

Unpaid or overdue payments may be referred to external debt collection services
